The Health Savings Solution implementation process is designed
to ensure an accurate and timely set up of your employees’ HSA program. Your
assigned U.S. Bank Health Savings Solution Implementation Specialist will provide
the personalized service you deserve and will work with you every step of the
way.
Our proven four-step process ensures your company's HSA implementation
with U.S. Bank Health Savings Solution is quick, easy and accurate.
Once you have selected U.S. Bank to provide HSA administration
services for your HSA Program, please complete the following three
(3) forms, print and sign (where applicable), and send to U.S. Bank.
Client Profile Questionnaire :
This document captures important client contact information and communicates
the implementation methodology for managing the enrollment and contribution process.
Health Savings Solution ACH Agreement : This agreement states that you, the Employer, authorize U.S.
Bank to initiate an ACH debit from your designated company DDA to fund contributions
you submit to U.S. Bank.

Once U.S. Bank receives
the above materials, a U.S. Bank Implementation Specialist will contact
you to discuss next steps.

Step
2 Employer contact by HSA Implementation Specialist
The successful administration of your employees’ Health
Savings Accounts (HSA) requires teamwork between your organization and U.S. Bank
during the implementation process. We are committed to making your experience
of establishing HSAs with U.S. Bank a positive one. That is why U.S. Bank assigns
you with a dedicated Implementation Specialist to guide you through the process.
Once U.S. Bank receives the three (3) documents detailed
in Step
1, you will be contacted by your assigned specialist within five (5) business
days to begin the implementation process.
U.S. Bank will work with you to ensure the process progresses
on time. The U.S. Bank Health Savings Solution
Implementation Specialist will assist you with the following during the implementation
process:
Manage the implementation of your HSA Program with a proven
process
Coordinate the establishment of your employees’ HSAs
Work with you to answer your questions and resolve any issues
Prepare and deliver employee enrollment kits
Work with you to determine the most efficient and secure
method of transferring data to U.S. Bank
Coordinate with your payroll provider (if applicable) to
ensure all data is provided in an acceptable format
Collect completed enrollment forms (if applicable)
Work with U.S. Bank to initiate the transmission of contribution
data as detailed in Step 4.
Start payroll deductions and remit them to U.S. Bank
in a timely fashion
